Dr. Pham is a double-boarded physician who implements mission-driven improvements and evidence-based behavioral health & medical integrated care at the population level. She is the Founder and President at Metalyst Health and is leading a team in Behavioral Health services and consulting engagements.

At Change Healthcare, she collaborates with hundreds of managed care organizations (MCOs) and hospital systems to solve systemic issues, particularly through utilization management, care coordination, claim analysis, behavioral health technology, operational efficiency and consulting engagements. As a former Chief Medical Officer at Institute for Community Living (ICL), she led integrated services in over 100 programs, such as translational shelters, supportive housing, ACT & IMT teams, Care Coordination, HCBS/CORE, Home Health, Outpatient levels of care, etc.

In addition, she is currently an Assistant Professor at Columbia University, a peer reviewer at the Substance Abuse and Mental Health Services Administration (SAMHSA) and a consultant to the Office of Mental Health of New York State & Connecticut Department of Public Health
